TO ALL PARENTS OF NORTHERN LIGHTS AND STARLIGHTS 2014-2015:
In order for your child to participate in camp next week, we are requiring the following be turned in BY MONDAY when camp starts:
-$20 transportation fee (ck to Northridge High School)
-Directory Information Form
-Emergency Information Form
-Food Allergy Form
-Code of Conduct Form
(Note: Forms were given out at the awards/orientation night in May and at the Icebreaker this past June. Please let me know if you didn’t receive them).
Again, these MUST be turned in by Monday when your student comes to camp. We are now going to be following what the athletic department does. We have to do this for liability and to make sure we get things taken care of for the front office in a timely manner.
